Several months since the tragedy of Raccoon city the world was forced to put the blame on former S.T.A.R.S. members Jill, Barry, Rebecca, and Chris Redfield. Believing that the large corporation named Umbrella was innocent from the terrorist cell group, the world labeled the Raccoon city innocent as an isolated occurrence and returned to its own agendas. But their ignorance was ill placed.

Far from the ruins of Raccoon city, in a rural town called Riverside, sits Umbrella's main base of operations. With the virus sample lost and most of their research destroyed in the incident, Umbrella has been rebuilding and making up for lost time in the underground complex below the city. Unknown to the residents of Riverside, a deadly viral lab sits below their beloved town and the risk of a biohazard, while never predicted, mirrors that of Raccoon city.

Intent on taking out the remaining branches of Umbrella, the renegade STARS team that survived Raccoon city have set up base in Riverside. With nobody to trust and loyalties in question, the renegade STARS have virtually dropped off the grid. Various STARS divisions located within the USA have been sent to hunt down the remains of their former comrades, believing that the renegade STARS were responsible for the leak in Raccoon.

But the betrayal of STARS is the least of the group's worries. A series of bizarre murder cases have occurred within the city, the victims held in the local hospital for further examination. While the world remained isolated from these connections, the renegade STARS know all too well what this means. In order to stop Umbrella and another Raccoon city from occurring the renegade STARS will have to give it their all.
